The most dangerous railroad crossing in the country is in Phoenix. It lies just a few miles northwest of downtown. From 2006 to 2015, the crossing had the highest number of reported incidents with 24, resulting in two injuries and no deaths.
I couldn't find any more recent data, but the level crossing near the Deer Park seems to be in the news at least once a year.
